RELEASE OF LIABILITY, WAIVER OF CLAIMS

By signing the document or paying your registration fee you. You wave or give up certain legal rights, including the right to sue  Ottawa Basketball Challengers or any of his representative or claim compensation following an incident.

Please, print, sign and bring with you the registration form with you to the first day of activities otherwise you will be denied participation

 

Protective Equipment and Rules

 

I have been advised to wear all protective equipment that is required by the rule and regulations of the governing body for Outdoor basketball as required by Ottawa Basketball Challengers ant follow any rules set out by OBC.

 

Assumption of risks

 

I’m aware that outdoor or indoor basketball involves many risks, dangers, and hazards, including but not limited to: loss of balance; difficulty or inability to control one’s speed and direction; variation or steepness in terrain; rapid or uncontrolled acceleration; failure of equipment collision with others players, coaches, participants.

 

 I’m aware of the risks, dangers, and hazards associated with Basketball and I FREELY ACCEPT AND FULLY ASSUME ALL SUCH RISK, DANDERS, AND HAZARD AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, PROPERTY DAMAGE OR LOSS RESULTING THEREFROM.

 

This Release Agreement and any rights, duties, and obligations as between the parties to this Release Agreement shall be governed by and interpreted solely in accordance with the laws of the province where the basketball takes place and no other jurisdiction.

 

I CONFIRM THAT I HAVE READ AND UNDERSTOOD THIS RELEASE AGREEMENT PRIOR TO SIGNING IT AND I’AM AWARE THAT BY SIGNING THIS RELEASE AGREEMENT I ‘AM WAIVING CERTAIN LEGAL RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN EXECUTORS, ADMINISTRATORS, ASSIGNS AND REPRESENTATIVES MAY HAVE AGAINST THE RELEASEES.
